HOW CAN WE HELP YOU RETURN HOME?

Nursing homes used to be the only option for people who needed help with their care. Now with the help of A&D Community Transition Services, current nursing home residents can return home or to the setting of their choice.
Our Transition Navigators will meet with you at the nursing home to help you achieve a successful discharge into the community. A care plan is developed with you and your allies to provide you with ongoing help after your return home. Assistance is even available to help you find and secure housing, purchase needed furniture and household items, or optimize your existing home environment.

Through person-centered planning, the Beneficiary and Navigator determine which services are appropriate and needed to support successful home and community-based living.
Participants can receive the basic services covered by Michigan Medicaid. In addition, participants can receive the following services unique to the Transition Program:

Secure and Set Up Housing
Medical Equipment
Personal Emergency Response System
Personal Care Services (if qualifications are met)
Home Modifications
Non-Medical Transportation
Program Referrals: PACE, Waiver, Adult Home Help, Community Mental Health, etc.
